Restore/Reinstate a License

If you wish to restore/reinstate a license that has expired, please contact the board office for an application. If your license expired within the last 3 years, you are eligible to restore/reinstate by paying all back renewal fees and providing evidence of the appropriate continuing education.

If your license expired over 3 years ago, you will need to reapply for a new license and take the appropriate level examination. You may NOT reapply for a new license if your license expired less than 3 years ago.

If you wish to restore/reinstate a license that has under a disciplinary order or consent decree, you must contact the board office for an application.
