    SPECIAL NOTE: IF YOU WANT TO APPLY FOR ENDORSEMENT OF YOUR LICENSE FROM ANOTHER STATE, PLEASE REVIEW THE INFORMATION  FOR LICENSURE TO DETERMINE THE APPROPRIATE LEVEL EQUIVALENT TO YOUR LICENSE IN THE OTHER STATE.
